The
"Dates Served" column below, will list the Purple Heart recipients
Date of Death, (for KIA's if known), or Date Discharged from the Military
(DD-Form 214 Item 11D).
Any active member can request a Purple Heart biography update as
long as you submit the required proof of award. All VDHA KIA biographies also
will be updated accordingly.
Deceased members’ biographies will be updated upon request by
anyone, as long as documented proof that the deceased member was awarded the
Purple Heart is provided.
Please contact the webmaster with information and
proof from one of the sources below:
1. DD214 (Discharge)
2. DD215 (Discharge Update)
3. WD AGO - War Department Adjutant General’s Office Form
4. Service records showing the award Military orders of the award
Note: The actual Purple Heart medal or a Certificate of Award,
in and of itself, will not suffice for verification purposes without the
submission of one of the preceding documents.
